This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

How we may use your information

We use your protected health information to provide and coordinate your care (treatment), to bill and receive payment (payment), and to run our practice (health care operations). We may also use it to remind you of appointments.

Substance use disorder treatment records

To the extent that we have substance use disorder patient records about you that are subject to federal confidentiality rules (42 CFR Part 2), we will not use or share information in those records in civil, criminal, administrative, or legislative investigations or proceedings against you without (1) your written consent or (2) a court order and a subpoena. If we ever contact you for fundraising using Part 2 information, we will give you clear and obvious notice in advance and a choice about whether to receive those communications.

Your rights

• The right to see and get a copy of your records.
• The right to ask us to correct information you believe is wrong.
• The right to request confidential communications.
• The right to ask for limits on certain uses and disclosures.
• The right to a list of certain disclosures we have made.
• The right to a paper copy of this notice on request.

Our responsibilities

We are required by law to protect your health information, to follow the terms of this notice, and to notify you if a breach affects your information.
